The National Committee for Quality Assurance has ranked aBrooklyn based, multi-specialty medical group and all of its eightlocations as a level three Patient Centered Medical Home-itshighest designation – for implementing a new, more patient-friendlyapproach to healthcare.

This recognition exemplifies the commitment of the physicians andstaff of Preferred Health Partners has for improving andmaintaining the health of the people in the communities they serveby providing accessible and coordinated quality care, said LouiseMalone, executive vice president of operations and administrationfor Preferred Health Partners, which earned the accolades forputting into practice a method of patient care that emphasizescoordination and communication.

The new model enacted by the medical group aims to ensure that itshealthcare service is what patients want it to be. Patients aretreated by a medical team — which consists of a physician leader,a registered nurse, a licensed practical nurse and a medicalassistant. The comprehensive care and treatment of the patient iscoordinated by the team and the patient is an active participant inthe care that he or she receives. According to Preferred HealthPartners, this model helps to promote disease prevention andmanagement, which reduces healthcare costs.

Patients have the opportunity to address concerns directly to anymember of the care team about their illnesses or treatments, andthe team educates them about their individual circumstances, sopatients know how best to manage their condition and live ahealthier lifestyle.

Preferred Health Partners has also started using a fully electronichealth record system, which allows physicians to see a patient’srecords from any of the group’s eight locations, order lab testsand access lab results.

The system also allows physicians to generate prescriptions and torefer to specialists electronically, which saves time and enablesphysicians to have more time to care for patients.
